Welcome. If this is your first visit to our Clear Lake practice, you can complete your new patient forms - a short registration and a medical and vision history - before you come in. Fill them out from your own kitchen table, at your own pace. It is a small thing that makes a real difference. Here, an eye exam is unhurried on purpose: we take about thirty minutes, we talk, we visit, and we get to know you. Doing your paperwork ahead of time is what protects those minutes, so your appointment can start with your eyes and your questions, not with a stack of paper on a clipboard.

Before you arrive

Two things to complete, a few things to bring

Before your visit, there are just two things to fill out. The first is a short new-patient registration. The second is a medical and vision history. Take your time with it. Nothing needs to be printed or notarized ahead of time, and if you would rather fill the forms in with us at the front desk, that is perfectly fine too.

Any conditions or surgeries, the medications and allergies you have, your family's eye history, and the reason for your visit.

Bring on the day

On the day of your appointment, bring a few things with you:

  • A photo ID
  • Your vision or medical insurance card
  • Your current glasses, or a copy of your prescription
  • A list of the medications you take

We are an insurance-based practice. We work with both vision and medical plans, and we also see patients on a self-pay basis, with CareCredit available if that helps. Bringing your insurance card lets the front desk verify your plan before you sit down with the doctor, so there are no hold-ups once your visit begins.

This is usually the next question, and it is a fair one. We do not post a price list, because what you will pay depends on your insurance and on what your visit actually involves. We would rather give you a real number than a wrong one. Call us at (281) 488-0066 before your appointment and we will walk you through it in plain terms. Alongside insurance, we accept CareCredit and cash.
